Complete the Medical Information and Emergency Authorization Form (PDF Format) and submit during Pre-Camp. Must be signed by parent/guardian and NOTARIZED! Include a recent picture and a photocopy of your insurance card (front and back).

Complete the First Aid and Medication Authorization Form (PDF Format) and submit during Pre-Camp. Must be signed by parent/guardian.

Complete the Substance Abuse Student Pledge and Marching Firebirds Contract (PDF Format) and submit during Pre-Camp. Must be signed by student AND parent/guardian.

Complete the Wright State University Release Waiver (PDF Format) and submit during Pre-Camp. Must be signed by parent/guardian.

Parents, please complete the Online Upbeat Club Volunteer Interest Form and let us know your volunteer interests for the coming school year. The form includes the on-going activities/events required to support our band program each year. Be sure to watch this web site for specific volunteer opportunities related to fundraising.

Check the Uniform Announcements to make sure you know your assigned uniform fitting date/time and how to purchase gloves, shoes and berets. Percussion, Trombones, Trumpets, Baritones and Saxophones should bring two two-liter bottles of pop to Uniform Fitting Day. All other sections (including Guard) should bring two two-liter bottles of pop to Band Camp. Uniform Fitting days are at Lakota West on July 24 and 25!

Set your alarm clock for bright and early on Monday, July 28. Pre-Camp begins at 7:00 a.m. on July 28 at LAKOTA WEST!
